Many phone books now have online counterparts with search utilities. Also, the yellow pages should have a section for "medical clinics".

Well, duh. Actually, it depends on how it's submitted to them. If they are are an approved facility, and it's submitted as an "office visit" then the insurance company should pay all but the co-pay on it.
Also, many insurance companies have websites with their approved doctors on them - many doctors now have extended hours on certain days or weekend hours. You just need to spend some time looking.
Besides, as Lanya said (before she edited her post) - they can't stop you from taking a day off to go to the doctor. They're just not obligated to pay you for that time. The FMLA says they can't fire you for taking time off for medical reasons, either.
