Most likely: Ivanka points at a pictureto  a dressmaker and says"like this one, except a little more of this and a little less of that."

Then they tell the publicist she designed it.

Is there a designer Ivanka perfume for people who wish to smell like an Ivanka?
There was a Trump cologne, but not too many people wanted to smell like Donald.

There is no proof that this was the price paid. People adored Jacqueline Kennedy for her French fashion sense. She was the last real clothes horse to occupy the White House.Nancy Reagan may have spent more, but she was more secretive.
If a person spends their own money on things like shoes and clothing, I don't see a problem. The whole purpose of this $`138 dress thing was publicity for the rubes.
